State encourages parents, guardians to complete 2025-26 School Quality Survey

Hawai‘i Department of Education invites parents and guardians to complete the 2025-2026 Hawai‘i Department of Education School Quality Survey.

Feedback provided by those who complete the survey is invaluable, helping state education officials and school leadership assess as well as enhance the quality of education in Hawai‘i schools.

All responses will be kept strictly confidential.

Survey instructions

  • The survey takes about 8 to 10 minutes to complete.
  • Complete the survey in one sitting, as responses cannot be saved and resumed later.
  • You can respond to one survey for each child.
  • You will begin the survey by inputting your child’s first, middle and last name initials, plus your child’s grade level. (If your child does not have a middle name, input “#.”)
  • Most questions require agreement responses ranging from “Strongly Agree” to “Strongly Disagree.” Select “Neutral” if you neither agree nor disagree, and “N/A” if you are unsure or the statement does not apply to you.

There are 15 languages available in which to take the School Quality Survey. Translations can be accessed here.

Additional directions can be found in a letter from Hawai‘i State Superintendent of Schools Keith Hayashi announcing the survey that students took home in early January for their parents or guardians.

Contact Hawai‘i Department of Education at 808-307-3650 from 7:45 a.m. to 4:30 p.m. Monday through Friday with any questions about the survey. Neighbor Island parents and guardians can call toll-free at 855-276-5801.

Questions can also be emailed to osip-accountability.support@k12.hi.us.

“Your feedback about our schools is important to us,” says Hayashi’s letter to parents and guardians. “The survey measures your perceptions of your child’s safety and well-being, your satisfaction with the school and your level of involvement. Your responses are confidential and will support schools in their continuous improvement efforts.”

Click here to take the survey online. Parents and guardians can visit their child’s school to complete the survey on-site if they need internet service.

Anyone who prefers a paper survey can request one per child by calling 808-07-3650 or toll-free at 855-276-5801.

Deadline to submit a completed survey is March 12.

“Thank you for sharing your voice, and for supporting and helping to improve our schools!” says the state Education Department in the survey’s introduction.
