According to the preliminary report, at approximately 5:15 p.m., first responders received a call of distressed swimmers in waters west of the Hanalei Pavilion lifeguard tower.

In an attempt to assist two swimmers, Jakubowicz became distressed. The two swimmers made it safely to shore. However, Jakubowicz became unresponsive in the water and was brought to shore by local surfers.

On shore, first ocean safety lifeguards and fire personnel administered CPR. American Medical Response personnel continued advanced life-saving treatment on Jakubowicz while in transport to Samuel Mahelona Memorial Hospital, but he could not be revived.
