Individuals being vaccinated should bring a picture I.D. and an insurance card, if they have one. Insurance is not required: the vaccines are free, and no bill will be sent.

Minors ages 12-17 must be accompanied by a parent or guardian who will need to sign a consent form for the minor being vaccinated. The mobile vaccination van will return to these same locations three weeks later to administer second doses for those who get the Pfizer vaccine. For telephone assistance, call the Kaua‘i District Health Office at 808-241-3495.

The effort is a partnership between the County of Kaua‘i, the Kaua‘i District Health Office, Wilcox Health, and Hawai‘i Health Systems Corporation’s Kaua‘i Region.

Those in need of an auxiliary aid/service or other accommodation due to a disability, call 808-938-0938 or email blanca.baron@doh.hawaii.gov. Requests made as early as possible will allow adequate time to fulfill your request.
